Finding a photo cleaner that doesn't run on ads and trackers
Photo cleaners are quick to build and easy to make look effective, which is why the category is unusually ad-heavy — a shrinking storage number reads as success however many ads surround it. Search "photo cleaner" on either store and open the first few results. A full-screen ad before you've deleted a single file is common enough to barely register as unusual, and that's worth pausing on, because it isn't like this in every app category.
Why the category is so ad-heavy
Two things line up badly here. Building a basic duplicate finder is a well-understood project that a small team can put together quickly, so the store fills with near-identical apps competing on nothing but marketing. And the product itself produces a satisfying result almost regardless of quality — a big number, a progress bar filling up, a "you freed 2.1 GB" screen — which reads as success to a reviewer whether or not the app did anything careful underneath it.
Put those together and you get a category where the fastest way to make money isn't a better cleaner, it's more ad impressions per session inside an app that already looks like it worked. That isn't true of every app in the category, but it explains why the pattern is so common.
What comes bundled with an ad-supported cleaner
The ad itself is the visible part. The less visible part is what comes with it: showing ads almost always means an advertising SDK is installed in the app, and that SDK is a separate piece of software with its own access to the device, generally to attach an advertising identifier to your session so the ad can be targeted. That's a different privacy position from "scans your photos on the device," even when both are true of the same app.
It's also worth knowing that an advertising SDK doesn't need to be actively showing you something to be present and reporting. Ad networks can be switched on or off remotely, or simply have nothing to show on a given screen or in a given region, so the absence of a visible ad at any one moment doesn't mean the network isn't there. The privacy label is what actually tells you, not whatever happens to be on screen when you check.

What 'no ads' does and doesn't tell you
An ad-free app is a good sign, but it's one signal, not the whole picture — plenty of apps carry analytics with no ads attached at all, usually to measure engagement rather than to sell it. The privacy label check is the more complete version of this same question, and it's worth running on top of the ad check rather than instead of it.
